SMARTnet Maintenance Consent

By filling out the Cisco Customer Consent Form, you are allowing Skyline-ATS "read-only" access to the assets allocated to your company. Many times, assets that are integral to your infrastructure sit uncovered, unbeknownst to you. Conversely, there may be assets covered to the hilt that have long been removed from your system. By allowing Skyline this access, we can catch these potentially expensive errors and omissions.

The consent form needs to be printed, filled out and returned to Skyline via fax. The form is in PDF format.
